Today at Doctor’s Choice Pharmacy, my technician asked a great question while filling prescriptions:
“Rania, I’ve noticed some prescriptions are for Hydroxyzine Pamoate, and others are for Hydroxyzine HCL. What’s the difference between them?”

I smiled—this is one of those questions pharmacy students and even patients often ask.

“Both are forms of Hydroxyzine, which is an antihistamine used for things like allergies, anxiety, itching, and sleep issues. But the main difference is in the salt form and how they’re absorbed and used.

Here’s the breakdown:

Hydroxyzine Pamoate (Vistaril®)

  • More often used for anxiety or as a sedative.

  • Comes in capsule form and tends to cross the blood-brain barrier slightly more, making it a bit more sedating.

Hydroxyzine Hydrochloride (HCL) (Atarax®)

  • Commonly used for allergies, itching, or nausea.

  • Usually available in tablet or syrup form.

  • Slightly less sedating, more focused on antihistamine action.

My tech nodded, “So can a doctor substitute one for the other?”

“Not exactly,” I explained. “While they have similar effects, they’re not interchangeable without the prescriber’s approval because they may choose one based on whether they’re treating anxiety, allergies, or itching. Also, the way they’re dosed can differ slightly.”

Before we moved on, I added, “If a patient has trouble swallowing, we can always compound a liquid version of either one to make it easier for them!”

It’s always great when my staff takes the initiative to understand medications better—it helps us provide even better care and education to our patients.
