Today at Doctor’s Choice Pharmacy, I received a prescription for a compounded ketoconazole shampoo. A patient called shortly after and asked,
“Rania, my doctor prescribed this for my scalp psoriasis, but isn’t ketoconazole usually for fungus?”

I explained, “You’re right—ketoconazole is primarily an antifungal, but it’s also very effective for scalp conditions like seborrheic dermatitis, dandruff, and even some forms of psoriasis. It helps reduce inflammation, scaling, and flaking while also treating the underlying fungal component that can make these conditions worse.”

She seemed surprised. “So it does more than just kill fungus?”

“Exactly,” I said. “It has anti-inflammatory properties and can help regulate sebum production, which makes it great for calming irritated, flaky scalps. It’s also a common choice for folliculitis and even some types of hair loss.”

She asked about the difference between the over-the-counter versions and the compounded one.

Before hanging up, I added, “Just use it 2 to 3 times a week, leave it on for about 5 minutes, and rinse thoroughly. And don’t forget to follow up with a gentle, hydrating conditioner if you have dry hair.”

She sighed in relief. “I’m really hoping this does the trick. Thanks for explaining, Rania.”
