Today at Doctor’s Choice Pharmacy, a patient called after picking up a new compounded prescription for Estradiol 10 mg/mL cream.

She said, “Rania, my doctor told me to apply 0.1 mL twice a day, but the instructions say ‘2 clicks BID’—I’m confused. What’s a click?”

I reassured her, “Great question! You’re using a measured-dose applicator. Each ‘click’ dispenses a precise 0.05 mL of cream. So two clicks = 0.1 mL. When your doctor said 0.1 mL twice daily, they meant 2 clicks in the morning, 2 clicks at night.”

She laughed, “So I don’t need a syringe?”

“Nope, just the applicator that came with it,” I said. “It’s easy and clean, especially for hormone creams like this. Just apply it exactly where your provider instructed—often the inner thigh, arm, or lower abdomen.”

Then she asked, “Why compound estradiol anyway? Isn’t it already available in patches and pills?”

I explained, “Yes, but compounding offers flexibility. Some women don’t tolerate oral estrogen, or they need a very specific dose that isn’t commercially available. Compounding also avoids unnecessary fillers and provides bioidentical estradiol in a cleaner, customized formula.”
