“Hi, I have a patient dealing with uneven skin tone and fine lines. Can you guys compound a cream combining Vitamin C and Tretinoin?”

I smiled—this is one of my favorite types of requests.
“Yes! We absolutely can. In fact, it’s a great combination,” I replied. “Vitamin C is known for its brightening, antioxidant properties, while Tretinoin helps with cell turnover and collagen production. Together, they can target hyperpigmentation, fine lines, and overall skin texture.”

She asked, “Do you typically compound them in one base or separately?”

“We usually compound them together in one cream or gel base, customized based on the patient’s skin type and the provider’s preferences,” I explained. “We can also adjust the concentration of Vitamin C—commonly 10% to 20%—and Tretinoin—ranging from 0.025% to 0.1%.”

She wondered, “Is there any stability concern when combining them?”

“Great question! That’s why we compound it fresh and use a stabilized form of Vitamin C, often ascorbyl palmitate or magnesium ascorbyl phosphate. These forms are more stable and compatible with Tretinoin, ensuring both ingredients remain effective.”

Before ending the call, I added, “We can also enhance the formulation by adding hydrating ingredients like hyaluronic acid or soothing agents to minimize irritation, based on what you want for your patient.”

She sounded pleased. “That’s perfect! I’ll fax over the prescription today.”
